- SME1 Initial Medical document blank
- The SME2 is the Boxers Record Card which is returned to the Club/Boxer. It must be presented to the Doctor and supervisor at all events.
- SME3 Medical document is to be used after a boxer has received a Suspension period – BSL – SME3 Medical Suspension Form
- To be handed to Boxer by Doctor should the need arise BSL – Head Injury Card
- To be completed and handed to the Doctor prior to a pre-show/event medical – Pre-Post-Bout-Medical-Pro-Forma-11-19.pdf
- To be handed to the Doctor prior to a pre-show/event medical BSL – Updated Declaration of Non-pregnancy forms
- To be used by all Clubs and completed by all adults prior to training BSL – Fit to Spar & Adult Physical Activity Readiness Questionnaire
