Please find below an updated version of the SME1 Medical Examination Form.
Please note, Version 7 of the form will come into effect from Thursday 1st September 2022.
Any SME1 forms received into the Boxing Scotland Head Office that are signed off by the Doctor after September 1st 2022 must be completed on the Version 7 Form.
Any previous versions of the SME1 form that are signed off by the Doctor after September 1st 2022 will not be processed and will be returned to the club.
We will still continue to process the previous version of the SME1 forms that are received into the office after September 1st 2022, but only if they are signed off by the Doctor before August 31st 2022 and have been completed within the prior three months.
It is the responsibility of the club secretary to ensure that the correct form has been completed, not that of the doctor who completes the form.
