During the 19/20 Season Club Affiliation period, it has been noticed by the registrar that a large number of clubs are continuing to use out of date SME1s. (Boxers Medical Examination Form)
Some of these documents are as old as 2016. The Medical Commission updated this document at the Medical Commission Meeting in March and has been available to download since the 5th March 19.

It is vital that the correct medical forms are used in keeping with insurance documents. With this in mind from today 17/9/19 any other version than SME1 Version 2 dated 5/3/19 will be returned to clubs to complete with the correct documentation.
